Our practice is a participating provider with most major insurance companies. Please review the list below to confirm our participation with your specific plan.

If we are a participating provider with your insurance plan, our office will file your claim directly with your insurance company. As required by your plan, copayments, coinsurance, and deductibles must be paid at the time services are rendered.  Coverage varies by plan, the benefits listed are general guidelines. Your specific coverage may vary based on your individual plan, deductibles, and policy terms.

If we are not a participating provider with your insurance plan, payment in full is due prior to leaving our office.

At the end of your visit, you will receive an itemized statement for your records, along with a receipt confirming your payment.
